Help us deliver better Canberra primary health care services

CHN partnered with 4 local practices to deliver the Social Workers in General Practice Pilot. Social workers supported patients with counselling, navigating health care, and helping staff and patients understand their role. 

CHN commissions mental health services, including by funding organisations such as Medicare Mental Health and headspace to provide psychological therapies, psychosocial services and services for children and young people. 

The FDSV pilot supports primary health care professionals recognise, understand and respond to early signs of family, domestic and sexual violence (FDSV).

CHN partners with local organisations to offer care coordination and assistance to First Nations people living with chronic conditions, and improve cultural awareness in health care services.

The Primary Care Pilot helps primary health care professionals and their patients with complex health needs work together to centre care on the whole person, to manage their health needs.

CHN partnered with stakeholders including consumers and clinicians to co-design and develop a pilot of a home Breathlessness Intervention Service for the ACT, which improves independence and self management for people who experience breathlessness.
